What Is Population Health Management?

Population health management (PHM) is the practice of using aggregated health data to understand and improve health outcomes across a defined group of people, rather than treating individuals one at a time. It originated in, and is still most widely used by, healthcare providers and health plans managing entire patient populations.

How Does This Apply to Employer Wellness Programs?

Employers apply the same underlying idea at a smaller scale: using aggregated, de-identified data from things like health risk assessments and biometric screenings to understand health trends across the workforce as a whole, then designing programming that targets the risk areas that actually show up in the data, rather than guessing. This is different from clinical population health management in scope (a workforce, not a patient panel) but similar in method: aggregate the data, find the patterns, act on them.
