What Is a Wellness Stipend?
A wellness stipend is a fixed amount of money an employer gives employees to spend on approved wellbeing expenses, things like gym memberships, fitness classes, mental health apps, or wellness products. Employees typically get to choose how to use it within whatever categories the employer defines.
What Can Employees Typically Spend a Wellness Stipend On?
Common categories include gym and studio memberships, fitness equipment, wellness and meditation apps, massage or alternative therapy, and sometimes mental health services. The specific list is set entirely by the employer, so it varies significantly from one company to the next.
Is a Wellness Stipend Taxable?
Yes, generally. Because wellness stipends cover general wellbeing expenses rather than IRS-qualified medical costs, they’re treated as taxable income, added to the employee’s wages and subject to standard withholding, the same way a bonus would be. This is a genuinely common point of confusion, since the stipend feels health-related, but the IRS doesn’t treat it as a medical benefit unless it’s specifically structured and restricted to qualify as one.
How Much Do Employers Typically Offer?
Most sources report a range of roughly $500 to $1,200 per employee per year, though this varies widely by company size and industry. Smaller companies often start with a smaller monthly allowance, while larger organizations tend to offer more as part of a broader wellbeing strategy.
Is a Wellness Stipend the Same as an LSA?
Not quite, though they’re close cousins, and the line between them is genuinely blurry in practice. A Lifestyle Spending Account (LSA) is typically broader, covering multiple categories at once (fitness, family care, learning, home office, and more), administered through a defined account structure. A wellness stipend is usually narrower, focused specifically on wellness-related expenses, and can be run as simply as an allowance added to payroll. Both are generally taxable, so that’s not the distinguishing factor. In practice, many employers use “wellness stipend” and “LSA” almost interchangeably when the LSA is scoped narrowly to wellness alone.
